Overview

The 6S Audit is a tool that can help you understand the 6S issues in a target area of your home.  You can use the 6S Audit Form to quickly assess and organize the 6S categories of Sort, Straighten, Shine, Safety, Standardize, and Sustain broken down into line items based on targeted conditions.  By rating the target area in this way you will learn a great deal about 6S and have a baseline of performance measurement for the targeted area.

  • Determine a future date for a second assessment
  • Identify areas of improvement

Steps to Complete

  1. Choose 6S participants responsible for the 6S Audit
  2. Print the 6S Audit Form
  3. Enter the date of the 6S Audit
  4. For the entire target area, enter the score in the box next to each line item
  5. Determine how many problems exist for each line item
  6. Enter the 6S score based on the 6S Audit Scoring Guidelines for each line item
  7. After rating each line item, add all the lines for each 6S category
  8. After rating each 6S category, add all the lines and enter the total 6S Audit score
  9. Document Areas of Improvement based on score lower than 3

6S Audit Scoring Guidelines

Scoring Key
Very good (0 problems or not applicable) = 4 Points
Good (1-2 problems) = 3 Points
Okay (3-4 problems) = 2 Points
Poor (5-6 problems) = 1 Point
Very Poor (7 or more problems) = 0 Points
